Record your healthy resolutions with this handy diary.

Write your monthly resolutions in the handy  planner below – one for food, one for activity. Come back to the plan on the last day of each month and refocus on that next month’s resolution.

You can get some resolution inspiration from our health professionals’ tips in How to stick to your New Year health resolutions – read through them and find the one(s) that appeal to you. Or make up your own, just be sure to keep them small and achievable (such as ‘walk to work’) rather than grand resolutions (like lose 20 kilos!).
